Welcome! In order for ministry to happen safely, we require all of our participants to have our annual forms on file. Before your child can participate in any of our ministry events, you must submit these forms either electronically or with a physical copy.
Below, you will find lists of all the ministry opportunities we provide throughout the year and a link to the annual forms for high school and middle school students, respectively. Even if you have filled out these forms in a past, new forms must be submitted every academic year.
